- Financial Aid Adjustment Form: Must be completed to reinstate aid previously canceled either by the student or the Fin Aid office, or to award unused funds to the summer term.
- Fresh Start Initiative Acknowledgement (fillable pdf) - Defaulted borrowers will be notified by Dept. of Education of their possible eligibility under Fresh Start. Defaulted borrowers applying for Title IV aid must sign and submit this notarized acknowledgement. - Selective Service Status Letter (fillable pdf) Males who did not register for Selective Service before their 26th birthday should complete the Selective Service Status Letter (pdf) and provide acceptable documentation to support their statement. Provide copies of both to the Financial Aid Office before submitting to the Selective Service System. SSS will send a letter to student regarding eligibility; student should then provide that letter to FA Office.

- Work-Study Application (online form): Students with financial need may be eligible for on-campus, federally funded jobs.
