Traditional General Scholarship Application (For brand new high school graduates - Due April 1)

Non-Traditional Scholarship Application (For anyone other than a brand new high school graduate - Due April 1)

Texas Grant - Statement of Student Eligibility (Available by signing into your Campus Connect account, click on "Student Information," scroll down and click on "Texas Grant")

Financial Aid Literacy Counseling (Available by signing into your Campus Connect* account, click on "Student Information," scroll down and click on "Financial Aid Literacy")*

Loan Request Form (Available by signing into your Campus Connect* account, click on "Student Information," scroll down and click on "Loan Request")*

Consent for Release of Information

Proof of Income Form (Must be completed if income source is other than earned wages and a tax return was not filed.)

Proof of Marital Separation (pdf) (Must be completed if independent student is separated or dependent student's parents are separated)

Request form for a Tax Return Transcript (4506T-EZ) (Above link will open up a PDF file that you can complete online or print it off, fill it out and mail to the IRS.)

Student Request to Cancel Financial Aid (pdf) (Must be completed if income source is other than earned wages and a tax return was not filed.)

Confirmation of Cancellation of Financial Aid (pdf) (Must be completed by a Financial Aid Counselor at the previous college you attended. Financial Aid will not be reviewed for eligibility until we receive this completed form.)

Bachelor's Degree Confirmation Form (pdf) (Must be completed if a student has earned a Bachelor's Degree or if student accidentally entered incorrect information on FAFSA.)

Loan Change Form (pdf) Must be completed if a student wishes to have loans because: Loan reinstated after previously being declined Loan cancellations Loan changes

Concurrent Enrollment Form (pdf) (Only for students enrolled in two colleges at the same time.)
